The next step to the Self Assessment process is attending the September 24th workshop with your team.  All team members attending must register through this online registration system. 

You should:

1. Come prepared to complete the Evaluation Team Record Review process. If you have a small district, you may want to bring your IEP samples to work on as well.

2. Before that date, please use the sampling process outlined in the manual to "draw" your samples of students to be included in the record review with regards to evaluations and IEPs if you want to work on the IEPs then as well. There would be a separate sample for evaluations and a separate sample for IEPs. 

Please bring the following:

1. Your binders from the May meeting that have been updated with the 2014-2015 materials from the DPI website, the individual student records for student names that were determined via the sampling process (i.e. complete IEP, including evaluation paperwork (R-1, EW-1, 1-3).

2. Copies of the standards and directions for each person reviewing records

3. Blank copies of the evaluation record review forms (enough copies for all students in the sample, plus about 5 extra copies)

4. Evaluation Recording Form (and IEP recording forms for each student, if IEP sample has been drawn, and if time permits).

5. Remember, when notified by DPI that 2014-2015 materials are available, replace items in your binder.

If you already completed the evalution portion of the self-assessment locally, there is no need to attend.

If you need special accommodations, registration must be made three weeks before workshop.

 


Cancellation Policy: Registered participants will receive a refund if they cancel prior to 3 working days before an event, minus any material costs that have already been incurred. Cancellations must either be made on MyQuickReg or by emailing cancel@cesa2.org. Because of our commitment to the presenter and the food vendor, participants who cancel less than 3 working days prior to the event or who do not attend the event, will be billed the full fee. CESA 2 reserves the right to cancel any event. Registered participants will be notified via phone and email if a cancellation occurs and will receive a full refund. 

Inclement Weather Policy: Every effort will be made to hold an event. CESA 2 will notify participants via email as soon as possible if an event has been cancelled due to weather conditions.

Disclaimer: CESA 2 will do its best to accommodate general dietary requests; if your dietary need is very specific or severe, you may want to bring your own lunch to ensure your comfort and health. CESA 2 does not assume liability for adverse reactions to food consumed or items one may come into contact with while eating at a CESA 2 event. If you have a dietary need or food allergy, please go into your MyQuickReg profile and update the special accommodations section. Notice of a special dietary need must be made 2 weeks in advance of the event.

By registering for an event with CESA 2, participants agree to grant to CESA 2 and its authorized representatives permission to record digital photos and/or video, of their participation. Participants further agree that any or all of the material photographed may be used, in any form, as part of any future publications, brochure, or other printed materials used to promote CESA 2, and further that such use shall be without payment of fees, royalties, special credit or other compensation. In-Person Events: Restrictions on our in-person events due to COVID-19 have been lifted. We will be responsive to what's happening within our communities, and guidelines from the CDC and local health agencies for future offerings and will update guidelines when necessary.
