13th Annual CESA 5 School-Based Speech-Language Pathology Institute
March 9 & 10, 2015
Fee increase to $275 after January 30, 2015
Purpose: This two-day conference provides speech-language pathologists (SLPs) hands-on, ready-to-replicate strategies and techniques to benefit the students they serve with speech-language disabilities. The conference also provides an opportunity for SLPs to re-connect with the greater community of school-based SLPs and to facilitate a cross-pollination of service delivery ideas. In addition to distinguished key-note speakers, this conference features vendor exhibits, breakout sessions, an ethics presentation and door prizes.
Each year nearly 400-500 SLPs attend our conference. Please join us in 2015!

SLP's and Educators are constantly looking for GOOD, appropriate books for students with significant disabilities. This fast-paced session will be a combo of “make-n-take” low to high tech books and exploring websites with options to either create your own book or read those that are already created. We will be using apps for the iPad and websites accessible to any platform, but resources and examples for the Chromebook and computer software will be shared. Participants should bring an iPad with permission/password to download apps. Have pictures available for creating your own books for students (student photos, school photos, choices, etc) in the iPad camera roll. Questions: email Judi cumley, cumleyj@cesa5.org

One Day Rates: We will offer one-day rates again this year. If you plan to attend on Monday or Tuesday ONLY, the cost is $165. Email slpinfo@cesa5.org and let us know which day you plan to attend and we will adjust your fee.
Student/Parent Rates: Special reduced pricing for full-time students/parents: $150 for both days. When registering, it will ask if you are a full-time student or parent. Please check "Yes" and we will adjust your fee.
Graduate Credit: Optional 1 graduate credit is available for 2-day attendees (additional fee). Please register for credit by checking the appropriate radial button below. This will ensure that you receive the course registration form in February. You pay the Graduate Credit fee when you check-in at the conference.
ASHA CMHs: Certification Maintenance Hours: Both days = 11 CMHs. This conference is not part of the ASHA CE Registry, which means it will not show up automatically in your ASHA CE Registry report. It is the participant's responsibility to submit a copy of the provided certificate of attendance with the other necessary documentation when submitting the 3-year Certification Maintenance renewal paperwork.

Our original lodging block has filled & the Resort is honoring State Rate rooms UNTIL February 28, 2015.

MORE DETAILS ABOUT LODGING AT GLACIER:
- If your organization is sales tax exempt in WI, please bring a completed Wisconsin S211 or copy of your Certificate of Exemption Status (CES) form and present it at check-in.
- If paying with a personal credit card, you'll also need a letter on district letterhead stating that your employer is reimbursing you for business related travel.
- If your district is paying by check, checks need to be received by the Wilderness 10 business days prior to the stay.
- The Wilderness will accept a signed purchase order for payment, but said payment must be received PRIOR to your stay.
- Bring the purchase order, S211 form or Certificate of Exemption Status (CES) form upon check in. This may be the best way to ensure that you are given tax exempt status.

Cancellations: Registered participants will receive a full refund if they cancel at least 10 working days before the conference. Because of our commitments to the venue, presenters, vendors and participants, no refund of any kind, for any reason, will be given for cancellation requests received after Monday, February 23, 2015. Registrations are transferable within your organization. Any lodging cancellations are the responsibility of the participant and not subject to any type of refund by CESA 5. CESA 5 WILL NOT CANCEL THE CONFERENCE DUE TO INCLEMENT WEATHER. This policy replaces the cancellation policy stated below.
