Registration Notes:

• Online Registration closes April 12, 2013. After this date individuals must register in person at the CREATE Conference April 25, 2013 as a walk-in registrant.
• Walk-in forms of payment are cash and check only.

April 25th Conference Registration Fees & Dates:

• Early Bird Registration (Feb. 1-April 2) : $100
• Late Registration (April 3-April 12) : $125
• Student Registration (Feb. 1-April 12) : $50 (must provide valid student ID on conference date)

Payment Policies and Options:

Please note that all conference dues must be received prior to registrants attending the CREATE Conference.

• All payments must be received no later than April 19, 2013.
• Walk-in registrants are accepted on-site, note that payment is due immediately at time of registering. Walk-in forms of payment are cash and check only.
• Non-paying registrants will not be admitted to the CREATE Conference.
• Online Registration Payment Options: Accepted forms of payment are credit card, purchase order, and check.
• Walk-in Registration Payment Options: Payment forms are cash and check only.

Refund Policy:

• Cancellations received Feb. 1 - April 12, 2013 will be given a full refund.
• Cancellations received April 13-20, 2013 will be refunded minus a $30 administrative charge.
• Cancellations received after April 20, 2013 will not be refunded.

Reservation Fees:

$70 - single

$80 - double

Reservation Notes:

•The room block for the CREATE Conference will be held until April12, 2013.

•CREATE Wisconsin does not make room reservations, individual registrants must contact the hotel and reference the CREATE Conference room block.

•Hotel reservations are an additional fee and are not covered in the CREATE Conference fee.

•Attendees must submit proof of tax exemption upon check-in on site.

•Refer to CREATE Conference when placing reservations.

Cancellation Policy:

Any registration cancellation must be received at least 8 business days before event in order for a full conference fee refund to be issued (full refunds do not include service fees. Service fees are nonrefundable).Registration cancellations received 1-7 business days before an event, a refund, minus $50 for each participant, will be issued. Persons who are registered, but not in attendance on the day of the session ("no call, no show") will be charged the full registration fee. CESA 6 reserves the right to cancel any session due to insufficient enrollment. Participants will be notified by email if a cancellation occurs.
