PDP Review Team Training Session

Sponsored by the CESA #4 PI-34 program and DPI
This training is designed for professional educators who are administrators, teachers, pupil services representatives, and IHE representatives who have completed the on-line PI-34 PDP self-assessment module http://www.uwex.edu/ics/PI34/.
All participants must bring their certificate of completion to the October 14th training. Once training is complete, your names will be added to the DPI website as a Certified PDP Review Team Member.

Where: Marie Heider Meeting Room, West Salem Schools, 405 East Hamlin Street, West Salem, WI
When: October 14, 2011, Registration: 7:30 a.m. Training: 8:00 a.m. - 4:00 p.m.
Why: To become a certified, active participant of the PI-34 licensing process and support your initial educators
Other:
- DPI will reimburse districts for substitutes if necessary!
- Continental breakfast, lunch, and afternoon refreshments will be provided
- Registration Deadline is October 10, 2011

Participant Cancellation Policy:
You must contact CESA 4 to cancel attendance one week prior to the scheduled event unless otherwise stated in the individual event details. CESA 4 will only provide full refund of registration fees depending on individual circumstances. Refunds depend upon the ability of CESA 4 to cancel any financial obligations created by the registration such as meals, materials, etc. CESA 4 reserves the right to cancel any workshop due to insufficient enrollment. Participants will be notified by e-mail or phone if a workshop is cancelled. If a participant is a 'no show' the school will be billed for the registration fee, or loss of the 'free' attendance slot.
Dietary Restrictions:
Because of the number of meals served, number of ingredients used, and food product changes from vendors, we cannot guarantee dietary restrictions as indicated in a user's myQuickReg profile will be accommodated. Participants with dietary restrictions such as food allergies should plan to bring their own refreshments.
Weather Conditions:
If a workshop has been cancelled due to weather conditions, every effort will be made to reschedule the workshop. If weather conditions are questionable, please call CESA 4 Voice Mail at (608) 786-4800; Toll Free (800) 514-3075. A message will be posted regarding workshop weather cancellations.
Additional Accommodations:
If you need an accommodation to participate including interpreter services (sign language or other spoken language) please email the workshop contact to set up these services. A two-week notice is appreciated.
